This form is for MLHPP providers to submit external educational credits to qualify for their value-based educational requirement. In order for external education to meet this requirement a CME credit certificate must be submitted to [email protected] (please see below for submission instructions). Any educational program should be value-based in nature and should contain components of the following definition of value-based:

Value-based healthcare is a healthcare delivery model that uses a holistic approach for improving patient health outcomes. Value-based care agreements utilize a payment structure that prioritizes value over volume by providing cost-sharing opportunities to providers who focus on helping patients improve their health, reduce the effects and incidence of chronic disease, and live healthier lives in an evidence-based way. These models require accurate capture of a patient’s risk to determine expected total cost of care, which is used as a benchmark to distribute shared savings. This payment structure strives towards the triple aim of reducing the total cost of care while improving patient experience and overall population health. Utilizing interdisciplinary approaches to communication, collaboration, and care coordination, value-based care emphasizes quality and health outcomes. It aligns care delivery with how patients experience their health and reconnects clinicians to their purpose as healers.

Some examples of value-based care topics include, but are not limited to:

  • Enhancing coding and documentation / HCC coding
  • Advanced care planning
  • Decreasing avoidable admissions and readmissions
  • Improving discharge to home
  • Reducing unnecessary variation in care
  • Chronic disease ambulatory care guidelines
  • Appropriate utilization of care
  • Patient-centered care
  • Optimizing Rx drug spending
  • Closing care gaps (e.g. cancer screening, diabetes care, immunizations)
  • Utilizing post-acute care
  • Palliative care during end-of-life
  • Referrals to PCPs for patients without them
  • National value-based guidelines
  • Order set use and compliance
  • Appropriate utilization of home care
  • Combating burnout
  • Care management for ED high utilizers
  • Safe disposition alternative to admission
  • Inclusion of care coordination and social work
