Reducing Hospital Readmissions: What Every Therapist Should Know

Advance your clinical expertise and learn practical strategies to reduce avoidable hospital readmissions in this engaging continuing education course designed for occupational, physical, and speech therapy professionals. Participants will review key clinical signs, symptoms, and presentations associated with conditions that carry a high risk for readmission and develop evidence-based treatment plans that incorporate appropriate interprofessional referrals. Upon completion, learners will be able to identify trends and contributing factors resulting in hospital readmissions, discuss risk factors affecting specific patient populations, recognize clinical indicators of high-risk diagnoses, and establish effective intervention strategies to improve patient outcomes.
