Main Line Health's Connection to Community can help you find transportation, food, housing and more — right in your neighborhood. Through a partnership with findhelp, Connection to Community serves as an online resource for free or reduced-cost local community programs offered by community-based health and social service agencies.

Connection to Community empowers you to find the assistance you need. And you can filter offerings based on location, age, gender, income and other criteria.

Making a difference

Through Connection to Community, you can find community-based programs that can help positively impact your health and wellness.

Recently, a 25-year-old female patient who was seen at the emergency room at Lankenau Medical Center, part of Main Line Health, requested help getting connected to outpatient substance use counseling. Through Connection to Community, she was referred to Helping Opportunities for People's Empowerment (HOPE). HOPE offers reduced cost programs for individuals with behavioral health challenges, co-occurring substance use disorders and HIV/AIDS infection. Notably, HOPE caters to individuals who speak Spanish as their primary language.

Another example is when Lankenau's Initiative to Improve Cardiovascular Access (LIICA) was assisting a 59-year-old male who requested assistance with food resources. A Main Line Health Community Health Worker referred the patient to Jewish Relief Agency's food delivery program. The Main Line Health team is working with the patient to complete the application to get him the healthy food resources he needs to improve his heart health.

Connection to Community is available to Main Line Health employees to assist patients as well as the publicly, for community members to find helpful resources on their own.
