It’s very important to let us know of any health and medical changes as well as changes to your personal information. Here’s when it’s time to call your transplant team:
- When your phone number, address, or insurance changes
- When your dialysis unit or schedule changes
- When you have questions about upcoming appointments that cannot be answered by information you already received
It’s also important to let us know:
- If you are unable to attend appointments that have been scheduled with us or appointments for pre-transplant testing
- If you are going out of town (vacation)
- If there is a change in your medical condition, you are hospitalized, or receive a blood transfusion
