PRIVATE-DUTY NURSING: Physical assessments, vital sign and medication monitoring, tube feeding and ventilator care.
Registered Nurse (4 hour minimum)	$45.00/hour
Licensed Practical Nurse (4 hour minimum)	$36.00/hour
Registered Nurse Assessment Visit  (1-2 Hrs)	$120.00
PERSONAL CARE: Bathing and dressing, medication assistance, home management, meal preparation, laundry and companionship.
Home Health Aide (4 hour minimum)	$ 19.00/hour
Home Health Aide (2 hours or less)	$45.00/visit
Live-In Home Health Aide/Companion	$230/night
Per Hour Add On Fee for Additional Client	$3.00/hour

NOTES:

• A Patient Statement of Account shall be invoiced to Client at least twice per month. 
• Payment in full is due within ten (10) days of receipt of Patient Statement of Account. 
• A $20.00 administrative fee is charged for a bounced check. 
• Transportation fees of $1.00 per mile apply when the Caregiver’s vehicle is utilized. 
• Any Caregiver parking costs associated with providing care to the Client, whether at Client’s location or associated with transporting the Client to another location, is the Client’s responsibility. 
• All Holidays (Jan 1st, Memorial Day, July 4th, Labor Day, Thanksgiving Day, Dec 24th Dec 25th, Dec 31st ) and any overtime (greater than 40 hours per Client per week) shall be billed at time and one half (150%) of the standard rate listed above. 
• Cancellation fees for established clients (2 hours of scheduled service) apply when scheduled services are not cancelled by Client or Client's representative with reasonable (4 hours) prior notice. 
• Cancellation fees for New Clients (Nursing Assessment Visit Charge) apply when a Nursing Assessment is performed prior to the start of care, and the case is cancelled by Client or Client's designated responsible party.
