Team

The Hospice team works with the patient’s own physician to identify patient needs and ensure those needs are met. The Hospice team includes:

Registered Nurses are the anchors of the hospice team. They are highly specialized in pain and symptom management. They assess and manage physical symptoms, and teach proper care techniques to the primary caregivers.

Patient’s Physician remains in charge of the patient’s care and becomes a part of the hospice team.

Hospice Medical Director acts as a resource to the team. He/she also serves as a backup for the primary physician to ensure quick response in emergency situations.

Home Health Aides help with the patient’s personal care needs, such as bathing and dressing.

Medical Social Workers provide counseling and support to the patient and family members and offer guidance in planning for changing care needs. They also help families explore additional care giving options and community resources.

Chaplains address spiritual needs during times of change, supporting and affirming each person’s spiritual identity, and respecting all faith traditions.

Trained Volunteers offer a variety of support services, everything from providing companionship for patients and caregivers, to shopping and running errands. The volunteers participate in an extensive hospice-training program.

Bereavement Specialists offer support for up to one year for family members dealing with the sensitive issues of grief and loss.
