Physician Certification of Care Plan
Established and Approved by a Physician
  • The Physician Certification and Plan of Care refers to the HCFA-485 form.
  • The Plan of Care (Plan of Treatment) must contain:
    • all information pertinent to the patient’s condition 
    • orders specific to the type of services to be provided
    • who will provide the service
    • how often the service will be provided
    • the length of time the service is to be provided
  • Orders for care must be obtained prior to performing the service.
  • All orders must be signed before the final bill is submitted to Medicare for payment.
  • The Plan of Care requires the attending physician’s signature and is reviewed at least every 60 days
  • Services provided after the Plan of Care expires (on the 61st day, after the end date listed on the 485) cannot be considered provided under a Plan of Care.
