The Survivorship Program at the Cancer Center of Paoli Hospital
The Path to Survivorship
The Survivorship Program at the Cancer Center of
Paoli Hospital is designed to help you transition to life following treatment.
Patients that receive Survivorship care often report less stress, have a
sense of control in their health and well being, can collaborate with
their healthcare providers to prevent disease, and know how to monitor
for signs of recurrence or indentify signs of late treatment effects.
They also report an overall improvement in their quality of life.
Survivorship care can help answer many common questions:
Who and how often should I see my medical providers?
Who should I call if I have a concern or a physical problem?
How can I prevent this from coming back?
How can I prevent other cancers?
Will I have late side effects of my treatment?
My other medical providers keep asking questions about my
treatments, but I can’t remember everything. Is there a document
that has this information?
Our Survivorship program will empower you to begin the healing process
Creating a personalized Survivorship care plan and summary of your treatments
Identifying late and long-term side effects of treatment
Reviewing cancer follow-up care
Reviewing preventative care strategies
Helping you start a “new normal”
