The Bryn Mawr Family Medicine Residency Program received certification
as a NCQA Level 3 Patient Centered Medical Home. This is a
tremendous accomplishment and identifies our residency program as
being on the cutting edge of the transformation of primary care in our
area.
Level 3 is the highest level of achievement. It is
based on 6 standards which include:
-
Enhance Access and Continuity
-
Identify and Manage Patient Populations
-
Plan and Manage Care
-
Provide Self-Care Support and Community
Resources
-
Track and Coordinate Care
-
Measure and Improve Performance
In order to qualify as a NCQA medical home, an office must
significantly redesign the way care is delivered, job
responsibilities, communication and follow-up with patients, as
well as numerous policies, plans, and
workflows. It involves the cooperation and input
of all staff as well as physicians and
requires cooperation, teamwork, and most importantly
excellent leadership.
