Refer a Patient

Referrals to Bryn Mawr Rehab Hospital can be made by a physician, case manager, social worker or family member. Our clinical liaisons are available on-site at many acute care hospitals throughout Pennsylvania, New Jersey and Delaware to review and evaluate patients for acute inpatient rehabilitation. We treat patients who can benefit from acute rehabilitation and ongoing medical management with the following diagnoses:

• Amputation

• Multiple trauma

• Arthritis

• Neuromuscular disorders

• Brain Injury

• Orthopedic conditions

• Bilateral joint replacement

• Stroke

• Burns

• Spinal cord injuries / illnesses

• Degenerative neurological disorders (GBS, MS, ALS)

• Vasculitis

• Hip fracture

We also treat patients with the following diagnoses, if acute inpatient rehabilitation is appropriate and medically necessary: 

• Temporal arthritis

• Polymyalgia 

• Lupus

• Rheumatica

• Polymyalgia Rheumatica (PMR)

• Gouty arthritis

• Abdominal aortic aneurysm

• Coronary artery bypass graft (CABG)
