Lankenau’s Congregational Nurse Program is a unique collaboration
between Lankenau and two faith communities in its service area; Christ
Lutheran Community Church in Upper Darby and St. Matthew A.M.E. Church
in West Philadelphia.
The goal of the program is to enhance the health status of citizens in
Lankenau’s service area through outreach that is led by a registered
nurse at each site. Duties include health education through the
coordination of seminars, workshops and support groups, many which
feature physicians and other health professionals from Lankenau;
personal health counseling; acting as a referral source to help people
utilize community resources and services, including referrals to
Lankenau physicians and the Clinical Care Center, when appropriate;
making home/hospital/nursing home visits and providing wellness
screenings clinics. This last duty has recently been leveraged by the
creation of the “Wellness Center Concept”.
Building on the Congregational Nurse model, we have instituted monthly
drop-in programs which feature screenings, “Ask the Doctor”, health
education and referral services. The goal is to solidify the reputation
of the church sites as community health resources, strengthen referral
linkages to the hospital, and train community health advocates to work
with the Congregational Nurses.
