Sub-Acute Care in a Hospital: A Place to Keep Healing After a Hospital
Some patients cannot return home immediately after a hospital stay
because they need additional nursing care while they recuperate. This
care, referred to as "sub-acute inpatient care," is often a transition
from hospital to home.

Rehabilitation: Helping People Regain Their Lives After Injury, Accident
The focus of rehabilitation is to help patients resume their activities
after they have suffered a stroke, brain injury, cardiac problem,
amputation, or other injury or accident, or if they have undergone
surgery. Services are offered in the hospital, on an outpatient basis or
at home. Services may include physical, occupational or speech therapy,
as well as help in using prosthetics or other assistive devices.
Rehabilitation professionals also offer home visits to evaluate the
safety of the home and recommend modifications and equipment as needed.

Skilled nursing facilities are for patients who have chronic health
problems and need short- or long-term medical care. Skilled nursing
provides this medical care in a safe, supervised environment where
