Information

The Bryn Mawr Rehab Hospital Day Treatment Program is a progressive outpatient rehabilitation program that is based on the philosophy that rehabilitation must take place in an environment in which the patient needs to function. The program helps patients attain their community, vocational and/or school reentry goals in their own environment.

The traumatic onset of an injury or illness can lead to permanent impairments and problems as patients interact in their real-life environments. When these difficulties are identified, interventions are developed and impelemented to overome the problems.

A unique strength of our community integration program is its focus on patient and family involvement and participation in treatment planning and goal setting. The patient is viewed as an individual with unique internal and external environmental influences. With our interdisciplinary approach and high level of patient/family involvement we are able to develop individualized treatment plans and provide therapy in the individual's own environment as needed.

The program adds a key component to the continuum of services Bryn Mawr Rehab Hospital offers. Patients may utilize our full continuum of care during their recovery process.

Who is Appropriate for the Program?

The Day Treatment Program is appropriate for individuals who:

  • Have an acquired disability that interferes with activities of daily living
  • Are limited in their ability to function at home, work, school or in the community
  • Are medically stable and able to tolerate a comprehensive therapy program
  • Are able to live safely at home with family support
  • Have school, work or community reentry goals

Upon admission to the program, the Day Treatment therapy team meets with the patient to create an outcome-based treatment plan that focuses on overcoming barriers and achieving identified goals. These goals are modified throughout the patient's rehabilitation course with both team and patient/family input.

Conditions Treated

Conditions treated include, but are not limited to:

  • Brain injury
  • Spinal cord injury
  • Stroke and other neurological conditions
  • Cancer (chemotherapy or radiation)
  • Multiple skeletal traumas

Services Provided

Individual treatment sessions and therapeutic groups are facilitated by clinicians. Treatment occurs within the program area and in the community where the person needs to function. Based upon need and family/patient goals, the plan will provide for therapies to overcome problems in the areas of personal care, community mobility/problem solving, work and/or school, parenting, housekeeping, budgeting/financial management, social interactions, healthy lifestyle, leisure activities and shopping.

Program Convenience

The Day Treatment Program offers a full or half day program. Patients may attend a full day from 9 a.m. to 3 p.m., just a morning or an afternoon  half-day session. This schedule enables caretakers to better manage their personal and professional responsibilities.

Patients participate in therapies that are provided in an environment similar to one in which the patient is expected to perform them. Individualized treatment plans are designed to help patients adapt to their environment and achieve their goals. Our positive outcomes and successful ratio of patients returning to their primary life roles reflect the success of these treatment plans.

Referral Process

A physician referral is required for admission to the program. A clinical screening assessment will be conducted to determine whether the patient will benefit from Day Treatment services. To initiate a referral, call the Day Treatment case manager at 484-596-7880.
