The Case Manager, Hospice, is responsible for planning and coordinating the care for the Medicare-certified Hospice patients. The Case Manager will conduct initial client assessment, screen patient for service eligibility and obtain necessary orders. The Case Manager will facilitate the clients progression on the critical pathway or care plan by coordinating services to be received in the home by the Hospice Interdisciplinary Team. The Case Manager will market the Medicare Certified Hospice Program to physicians, discharge planners and other referral sources, as well as to the community.
Graduate of NLN approved school of nursing.
Bachelors degree preferred.
Current license to practice nursing in Pennsylvania.
Valid driver’s license and automobile insurance.
Current CPR certification is required - CRNH preferred
Three to five years nursing experience with at least one years
experience in Hospice and one year recent experience in home
health or discharge planning.
Home health nursing.

Working knowledge of Medicare-Certified Hospice.
