Healthy Communities Partnerships is a collaboration of people, organizations, government and businesses working together to continually improve their economic, cultural and
environmental health surrounding. The combined resources of everyone involved will enable people and communities to achieve
their maximum potential and share in a higher quality of life. The Community Health Departments of Main Line Health's hospitals
-- Bryn Mawr, Lankenau, Paoli and Bryn Mawr Rehab -- partner with the following communities by providing financial assistance,
staff expertise and health education, prevention and screening if appropriate.
The Congregational Nurse Network (CNN) of Main Line Health The Congregational Nurse Network has provided start-up grants to area churches for Congregational Nurse Programs. Congregational
nursing programs have been successful in reaching congregations and surrounding communities, to provide holistic and preventive
healthcare and to help individuals access the healthcare system. This approach builds on the existing sense of “community”
in congregations and supports the concept that good health encompasses physical, emotional and spiritual health. Programs
provided at participating churches are open to anyone living in the community.
Chester County Healthy Communities Partnership The Chester County Healthy Community Partnership is committed to improving the quality of life in Chester County through the
collaborative efforts of concerned individuals and groups. The Partnership is focusing on encouraging program collaboration
among organizations that address the needs of youth in the county.
Chester County Safe Kids Coalition A non-profit coalition made up of a variety of organizations throughout Chester County dedicated to reducing unintentional
childhood injuries. More Info!
Community Volunteers in Medicine A non-profit community-based organization serving the primary health care needs of the uninsured and the underinsured population
in Chester County. More Info!
Montgomery County Healthy Communities Partnership, Norristown Initiative A collaborative with representation from all stakeholder groups. The goal of the Initiative is to make the Borough of Norristown
a better place to live, work and play and empower all sectors of the community to work together forming a dynamic force to
create change.
MOMobile MOMobile is a program of the Maternity Care Coalition (MCC). MCC is a non-profit organization that works to reduce infant
mortality and improve maternal and child health through outreach to high-risk communities and advocacy at the local, state
and national level. This community-based outreach program uses mobile vans and door-to-door canvassing to identify pregnant
women in neighborhoods known to have high rates of infant mortality, teenage pregnancy, poverty, child abuse and neglect.
PALS, Partners in Accessing Life Services Nine agencies in Montgomery County have come together to serve the Senior population of the County. The goal is to assist
the elderly in meeting the needs that are not met through other agencies and institutions.
LM-NCC Lower Merion-Narberth Community Coalition LM-NCC is a partnership of community agencies and individuals working together to foster an atmosphere where children feel
safe, secure and included. It is made up of representatives from the Lower Merion Township Administration, Lower Merion School
District, independent schools, the business community, religious organizations, adolescent health professionals and our state
and federal government representatives as well as community members.
CASY, Community Advocates for Safe Youth CASY is a group of concerned community members who serve as the Lower Merion School District's Drug and Alcohol Council. Their
primary focus is the youth of the community and the issues that jeopardize their well-being. CASY supports and works in collaboration
with the Lower Merion-Narberth Community Coalition (LM-NCC) to provide comprehensive programming that gives consistent messages
to the youth of the community in a school setting and the community at large.
Bridgeport A collaborative effort with the Borough of Bridgeport Department of Health to assess the needs of the community, provide education,
screenings and resources, as identified. Clients are served in schools and in their community.
ElderNet Main Line Health assists this non-profit agency that serves the low income elderly of Lower Merion and Narberth. ElderNet
helps older adults remain independent and provides a variety of practical, free services so they can lead full, satisfying
lives.
Chester County School Partnership This program offers partnering school districts opportunities for educational programming, curriculum review and support,
along with health maintenance, promotion and prevention programs and activities.
