  • JeffPLUS PPO pays 100% of allowed charges if MLHL is used.
  • Aetna and IBC HMO patients: HMO patients must use their primary care physician’s (PCP) capitated lab for routine lab services to get maximum benefits. The patient may have specimens collected at an MLHL facility if the PCP’s capitated lab (Quest or LabCorp) requisition form is used.
