Hospital Specialties & Services Day Treatment Program
The traumatic onset of an injury or illness can lead to permanent impairments and problems as patients interact in their real-life environments. When these difficulties are identified, interventions are developed and impelemented to overome the problems. A unique strength of our community integration program is its focus on patient and family involvement and participation in treatment planning and goal setting. The patient is viewed as an individual with unique internal and external environmental influences. With our transdisciplinary approach and high level of patient/family involvement we are able to develop individualized treatment plans and provide therapy in the individual's own environment as needed. The program adds a key component to the continuum of services Bryn Mawr Rehab offers. Patients may utilize our full continuum of care of one or more service, in their recovery process. Services Provided
Patients participate in therapies that are provided in the environment in which the patient is expected to perform them. Individualized treatment plans are designed to help patients adapt to their environment and achieve their goals. Our positive outcomes and successful ratio of patients returning to their primary life roles reflect the success of these treatment plans. Referral Process
