  1. No Response
    • Appears to be in deep sleep
    • Completely unresponsive to any stimuli presented
  2. Generalized Response
    • Reacts inconsistently and non purposefully to stimuli
    • Responses are limited and often the same regardless of stimuli
    • Responses may be physiological changes, gross body movements, and/or vocalization
  3. Localized Responses
    • Reacts specifically but inconsistently to stimuli
    • May follow simple commands in an inconsistent, delayed manner
    • May show a vague awareness of self and body by responding to discomfort by pulling at nasogastric tube or catheter
    • May show bias toward responding to some persons but not others
  4. Confused-Agitated
    • Severely decreased ability to process information
    • Bizarre, non-purposeful behavior (may cry out or scream out of proportion to stimuli)
    • Verbalization is frequently incoherent and inappropriate to the environment.
    • Unable to perform self-care without maximum assistance
  5. Confused-Inappropriate Non-Agitated
    • Alert and able to respond to simple commands
    • Highly distractible and lacks ability to focus attention to specific task without frequent redirection.
    • Verbalization is often inappropriate
    • Can usually perform self-care activities with assistance
  6. Confused-Appropriate
    • Shows goal-directed behavior
    • Follows simple directions consistently and shows carryover for tasks relearned
    • Selective attention to tasks may be impaired, especially with difficult tasks and in unstructured settings
    • Increased awareness of self, family, and basic needs
